Half rels can occur for a slew of reasons. Even when using the proper perscribed methods they can happen under the right apocalyptic server conditions. 
Best bet is to stick to the reccomended facades and API and avoid overloading the server while removing or moving devices. Use of transact for stressful or repetative dmd ops are hugly helpful too.
